Please note that the Brisbane Eye Clinic (BEC) policy is that patients need to notify the BEC administration team of any of the following changes to their personal details (this list is not exhaustive);
- Residential address;
- Contact details;
- Postal address;
- Referrer Details ;
- General Practitioner Details;
- Other Specialists Details;
- Medicare amendments (including card number, expiration date or reference number);
- Private Health insurance details (including amendments to health funds);
- Enduring Power of Attorney; or
- Advanced Health Care Directives
This information must be delivered or communicated to the BEC administration office in writing, in person, on the BEC website or through a Carer by fax, email, post, phone or direct entry at our registration portals.
